Servers in the top-level mcpServers block of ~/.claude.json load in every working directory.

Fix

Register servers under each project's own block and keep the top-level block empty unless a server truly belongs everywhere.

Move entries from mcpServers to projects["/path/to/project"].mcpServers in ~/.claude.json, after backing the file up
