Discord followup sent as ephemeral is visible to everyone after a deferred reply

Cause

When a response is deferred, the deferral decides whether the final message is ephemeral. A followup that fills in the loading message ignores the ephemeral flag.

Fix

Set the ephemeral flag on the deferral itself.
