Cause

Only the first 200 lines or 25KB of MEMORY.md, whichever comes first, load at the start of a session; the rest is dropped. Topic files load only when Claude chooses to read them.

Fix

Keep MEMORY.md as an index with one short line per entry and move detail into topic files the index points to. Merge or delete stale entries.
