.mcp.json servers stay 'Pending approval' and enableAllProjectMcpServers in the repo is ignored

Cause

Project-scoped servers need approval, and a cloned repository cannot approve its own servers: approval keys in the committed .claude/settings.json are ignored until the folder is trusted. Since v2.1.207 an untracked .claude/settings.local.json is also applied only in a trusted folder.

Fix

Run claude once in the folder and accept the trust dialog and the server prompt, or put the approval in your user settings, managed settings, or a --settings argument, which apply even in an untrusted folder.
