Cause

The run died at pre-flight, before any module or connection was touched. Several scenarios that share no connection fail inside the same short window and each one succeeds on its next scheduled run. A truly expired token fails on every run, fails at the module with a 401, and uses at least one operation.

Fix

Check whether the next run of the same scenario succeeded. If it did, do nothing and do not reconnect healthy connections. In monitoring, suppress an isolated failure with this signature when a later run succeeded, and alert only when it repeats on consecutive runs.
